Texas Transition’s Next Steps to Independence: Skills and Strategies

This checklist from Texas Transition Student Centered Transition Network lets educators, families, and students monitor continued progress on skills needed for future success.  The checklist is broken into age categories:  Birth to Age 3; Age 3-5 (Including Kindergarten); Elementary School (Grades 1-5); Middle School (Grades 6-8); High School (Grades 9-12); and 18+ (Ages 18-22).  Each category includes a checklist, important actions steps for families, and teaching and learning strategies.
